Title Identifying the factors contributing to progression of posthemorrhagic hydrocephalus and assessing its impact on clinical and neurodevelopmental outcomes

Background Posthemorrhagic hydrocephalus of prematurity (PHHP) is a complication of intraventricular hemorrhage (IVH), characterized by the accumulation of cerebrospinal fluid (CSF) in the ventricles due to impaired reabsorption or obstruction. This leads to increased intracranial pressure and the release of iron degradation products, which impact the development of the central nervous system and immune system, contributing to neurological impairment. In preterm infants, transient, naturally resolving ventricular dilation (PHVD) often occurs within weeks after IVH, with some cases progressing to PHHP. However, research on the prenatal and postnatal factors contributing to this progression and the effects of each stage on neurodevelopmental outcomes (NDO) remains limited.
Aim / Hypothesis This study aims to investigate the prenatal contributing to PHVD or PHHP, the postnatal comorbidities associated with their development, and to assess its impact on neurodevelopmental outcomes
Inclusion Criteria Preterm infants with a birth weight of less than 1500 grams who were registered in the Korean Neonatal Network (KNN) from January 2013 to December 2023
Exclusion Criteria 1) infants with major congenital anomalies 2) infants who have died 3) infants without data on the presence or absence of ventriculo-peritoneal shunt placement at CA 18-24 months or 3-4 years
Study Design Statistical methods 1. Classification of Groups: In this study, infants diagnosed with PHH will be grouped based on low-grade IVH versus high-grade IVH, IVH grade, or the presence of a VP shunt. 2. Comparison of Factors: The groups will be compared in terms of prenatal factors, frequency of morbidities, and neurodevelopmental outcomes (NDO). 3. Statistical Methods: Appropriate statistical methods will be applied after reviewing the data, including mediation analysis, propensity score matching, clustering analysis or logistic regression, depending on the specific characteristics of the data.
